Admission guide

How Indian students get into medicine in Italy

Public medicine in Italy is open to non-EU students through the IMAT, an English-language entrance test held once a year. Ranking decides everything — there is no interview and no essay.

Entrance test

IMAT (International Medical Admissions Test)

60 questions in 100 minutes covering logic, general knowledge, biology, chemistry, physics and maths. Sat in September at test centres including several in India. Each university publishes a separate non-EU quota and a separate cut-off, so your choice of university changes the score you need.

Language of instruction

English for the full six years at IMAT universities; Italian needed for hospital placements.

Who is eligible

  • Class 12 with physics, chemistry, biology and maths, 12 years of schooling completed
  • IMAT score above the chosen university's non-EU cut-off
  • Declaration of Value or CIMEA statement of comparability for your Class 12 certificate
  • NEET qualification if you plan to return to India

Application sequence

  1. 01

    Jan–Jun: register a Universitaly account

Pre-enrolment runs through the Universitaly portal and is tied to the university you rank first. Non-EU candidates must also book the IMAT seat through the Cambridge portal.

  2. 02

Jul: apply to the Italian consulate

Start the Declaration of Value or CIMEA route early — this is the step that most often delays enrolment, not the test.

  3. 03

    Sep: sit the IMAT

Results and university-by-university rankings are published within two to three weeks.

  4. 04

    Sep–Oct: seat allocation and scroll

Unfilled seats scroll down the ranking in rounds. Do not book flights until your name appears as assigned.

  5. 05

    Oct–Nov: study visa and enrolment

Apply for the type-D study visa with the pre-enrolment confirmation, proof of funds and health cover, then complete enrolment and the residence permit within eight days of arrival.

Documents to prepare

  • Class 10 and 12 marksheets and certificates
  • Declaration of Value or CIMEA comparability statement
  • IMAT score report
  • Universitaly pre-enrolment confirmation
  • Proof of funds and health insurance for the visa
  • NEET scorecard (for the Indian licence route)

What it costs (estimate)

Public tuition is income-scaled and often lands between €500 and €4,000 a year; low-income families can pay close to nothing. Living costs run €700–€1,100 a month depending on the city.

Licensing and practising

The Italian degree plus the state exam gives EU practice rights. For India, the six-year course with internship in Italy satisfies the 54-month rule, then you clear FMGE/NExT.

Where applications go wrong

  • Ranking a famous university first without checking last year's non-EU cut-off
  • Leaving the Declaration of Value until after IMAT results
  • Assuming English instruction means no Italian is needed for clinical rotations

True for every European medical route

  • NEET qualification is required if you ever intend to practise in India — the score does not decide European admission, but without it your Indian licence route is closed.
  • Medical degrees in Europe are 5.5–6 years and are legally titled MD/MBChB/Laurea Magistrale; India recognises them as equivalent to MBBS when the course is at least 54 months plus a 12-month internship in the same country.
  • Clinical years are taught in the local language almost everywhere, even on English-track programmes. Plan for B1–B2 in the local language by year three.
  • Transfers between European medical schools mid-course are rare and usually cost a year. Choose the country you can finish in.
