MBBS in Italy for Indian Students: How the IMAT Route Actually Works

By Counselling Desk, Senior counsellor, Italy medicine admissions · 11 min read · Updated 2026-08-20 · Editorial fact-check, August 2026

“Can I really do MBBS in Italy in English without paying donation money?”

Yes. Public Italian medical schools admit non-EU students purely on IMAT rank, teach the six years in English, and charge income-scaled tuition that is usually between €500 and €4,000 a year. There is no interview, no essay and no capitation fee anywhere in the process.

The route in one paragraph

You register on Universitaly and rank the English-taught medicine courses you want, book an IMAT seat through the Cambridge portal, get your Class 12 certificate validated (Declaration of Value from the Italian consulate, or a CIMEA comparability statement), sit the IMAT in September, and then wait for the ranking. If your score clears the non-EU cut-off at a university you ranked, you get a seat. If it does not, unfilled seats scroll down the ranking in rounds, and students move up. Only after your name is assigned do you apply for the type-D study visa.

That is the entire mechanism. Almost everything that goes wrong for Indian applicants goes wrong in document validation or in university ranking — not in the test itself.

What you need to be eligible

Eligibility is unusually simple compared with Germany or France, because Italy does not ask for a foundation year:

  • 12 years of schooling completed, with physics, chemistry, biology and maths in Class 11–12
  • An IMAT score above the non-EU cut-off of a university you ranked — there is no national pass mark
  • A Declaration of Value or CIMEA statement of comparability for your Class 12 certificate
  • NEET qualification, only if you intend to practise in India later

Where the conditions and exceptions sit

There is no minimum Class 12 percentage — Italy does not weigh your board marks at all. But quotas are per-university and per-category, so 'the cut-off' is not a single number: a score that wins a seat at Messina or Bari may be nowhere near enough at Pavia or Milan.

English instruction covers the taught years, but clinical rotations happen in Italian hospitals with Italian patients. Universities expect you to reach B1–B2 Italian by year three, and most run free or subsidised language courses. Students who ignore this find third year much harder than second.

NEET is not part of Italian admission. It matters only for the Indian licence route: without a valid NEET qualification for the year you started, the National Medical Commission will not let you sit FMGE/NExT when you return.

The sequence, with the dates that actually bind

The test date is fixed; the paperwork is what slips. Work backwards from September:

  • January–April: start IMAT preparation and begin the Declaration of Value or CIMEA process — the consulate is the slowest actor in the chain
  • April–July: Universitaly pre-enrolment opens; register the account and prepare your university ranking
  • July: book the IMAT seat through the Cambridge Assessment portal before slots in Indian test centres fill
  • September: sit the IMAT; results and per-university rankings appear within two to three weeks
  • September–October: seat allocation and scrolling rounds — do not book flights until your status reads assigned
  • October–November: type-D study visa with the pre-enrolment confirmation, proof of funds and health cover, then enrolment and the residence permit within eight days of arrival

What this costs in practice

Public tuition is scaled to family income through the ISEE-parificato assessment, so two students in the same lecture hall can pay very different fees. Low-income families often pay only the regional tax of roughly €150–€350; a mid-income family typically sees €1,000–€2,500 a year. Living costs run €700–€1,100 a month, with Milan and Rome at the top of that band and southern cities at the bottom.

Regional scholarships (DSU) are means-tested and can cover fees plus a maintenance grant, but they require the ISEE paperwork to be filed correctly in your first weeks. Students who skip it lose a year of funding they qualified for.

The mistakes we see every cycle

Three errors account for most failed applications, and none of them are about aptitude:

  • Ranking famous universities first without checking last year's non-EU cut-offs, and missing a seat by two marks
  • Starting the Declaration of Value after IMAT results, then missing the enrolment window
  • Assuming the English track means Italian is optional, and hitting clinical years unprepared

The next decision

Decide whether you can commit roughly six focused months to IMAT preparation before the September test. If you can, the next task is picking which universities to rank — that choice sets the score you need.

If that is where you are stuck, read compare last year's non-EU cut-offs before you rank universities, work out the real six-year cost of Italian medicine, or the Italy country hub with visa and living-cost detail.

Questions students ask next

Is the Italian medical degree valid in India?
Yes, when the course is at least 54 months plus a 12-month internship in the same country — the Italian six-year Laurea Magistrale in Medicine and Surgery satisfies that. You still need a valid NEET qualification from the year you began, and you must clear FMGE/NExT to practise.
How many attempts do I get at the IMAT?
One per year, and there is no lifetime limit. Many students who miss the cut-off by a small margin re-sit the next September, often with a stronger university ranking strategy rather than a very different score.
Do I need Italian to apply?
No. Application, test and the taught years are in English. Italian becomes practically necessary for hospital placements from around year three.
